Myocardial perfusion imaging is a nuclear medicine imaging technique that allows to make an approach to the coronary condition in patients with moderate probability to have coronary artery disease. 99mTc MIBI myocardial perfusion is one of the most common techniques in nuclear medicine clinical practice. Extracardiac accumulation of the tracer is relatively common, and can be seen in certain tumors with high metabolic rate, due to the radiotracer’s mitochondrial preference.

We describe the case of a woman previously treated for Lyme disease and babesiosis who relapsed with severe malaria-like symptoms during the 3rd trimester of two consecutive pregnancies.

It is a nonfunctional tumor that often presents as an infrequent mass, leading to potential misdiagnosis. Case presentation: This case report presents the case of a 77-year-old man who was referred to the hospital due to a severe productive cough and dyspnea. The symptoms had begun three weeks prior.

Primary extra cranial meningioma of the ear and temporal bone are exceedingly rare, making up less than 1% of all meningiomas. Histologically, they are indistinguishable from their intracranial counterpart. The histopathologic diagnosis is often challenging, and the differential diagnosis for neoplasms in this unusual location is quite extensive. We report a 64-year-old male patient with history of seizures who presented with coalescing mastoiditis and sigmoid sinus thrombosis.

The median age at diagnosis of multiple myeloma is 70 years, and the incidence increases with age.

Blastomycosis, formerly known as Gilchrist disease, Chicago disease, North American blastomycosis, or Namekagos fever, is an uncommon but underappreciated fungal infection seen mostly in immunocompetent people, caused by the fungi Blastomyces. Blastomyces dermatitidis and Blastomyces gilchristii are the most common species responsible for most infections in the United States of America. Blastomycosis can cause a wide variety of diseases, the most common being pneumonia as the lungs are usually the main primary entrance source of infection after inhalation of Blastomyces spores.

Right Atrial Metastasis from Melanoma Treated with Immunotherapy

A patient with metastatic melanoma is treated with immunotherapy and achieved a partial response. On treatment holiday a right atrial metastasis is diagnosed radiologically. While cardiac surgery has been a historic consideration to treat this the use of dual immunotherapy with ipilimumab and nivolumab is discussed. This can provide a meaningful response and potentially increased DFS. Despite a response, other metastatic progression can still occur and close surveillance for CNS metastases may be important in this setting.

Acute Abdomen due to Perforation of Small Bowel Malignant Melanoma Metastasis

Metastatic malignant melanoma of the small bowel is a rare and aggressive disease, without specific symptoms. Due to that, most cases are diagnosed in advanced stages, presenting as an acute abdomen in situations of bowel obstruct ion or perforation. We present the case of a 56-year-old male who was admitted for generalized edema and developed an acute abdomen secondary to perforation of small bowel malignant melanoma metastasis, with lymph node invasion.

As we continue to use these agents routinely, recognition of rare but real toxicities, such as cardiotoxicity, has also improved. The treatment options for patients who have encountered fluoropyrimidine-induced cardiotoxicity are limited as many anti-angiogenic drugs also pose a cardiac risk.

Red cell fragmentation may result in erroneously high platelet counts by automated blood counters. Therefore, any abnormal platelet counts in a full blood count analysis should be re-evaluated either using blood film manual counting or with more accurate automated analysis options. Misleading Clumsiness: Unraveling Confusion Between Balance and Visual Field Constriction Caused by Amiodarone

This study highlights a unique case of amiodarone toxicity, presenting with atypical symptoms of bilateral concentric visual field constriction and ambulation instability, without optic neuropathy or optic nerve edema. Despite no decline in visual acuity, the patient experienced significant visual field loss, along with gait instability, both of which resolved upon discontinuing amiodarone. This case underscores the importance of thorough baseline medication reconciliation and heightened awareness of unusual amiodarone-induced side effects impacting both visual fields and balance.

Double Trigger, Reverse Triggering, and Pseudo-Reverse-Triggering

Reverse triggering is the ventilator-triggered muscular effort, related to entrainment, particularly seen among sedated patient. The reverse triggering frequently caused double triggering/double cycling, with increased tidal volume and transpulmonary pressure. In our monitored patients we have observed different types of double triggering caused by asynchrony like to reverse triggering, for which we adopted the term “pseudo-reverse-triggering”.

Case Report of Pulmonary Embolism with Right Ventricular Strain in a Young Female

Acute pulmonary embolism (PE) is when one or more thrombus travel to the lungs and obstruct the pulmonary artery or one of the branches of the pulmonary tree, producing signs and symptoms immediately after the obstruction. Saddle pulmonary embolism (SPE) is a rare type of acute PE that can lead to hemodynamic instability and death. The incidence of pulmonary embolism increases with age. In women, the risk of PE increases with pregnancy, hormonal contraceptives, and hormone replacement therapy.

Resolution of Osborn Waves after the Complete Resolution of Spontaneous Pneumothorax in a Normothermic Patient

We report a case of Osborn waves occurring in a 55-year-old normothermic male patient with spontaneous pneumothorax that resolved with the resolution of the pneumothorax. The patient was presented with a 2 week history of shortness of breath, pleuritic chest pain, and cough. He was found to have a left sided pneumothorax that persisted even after chest tube placement.

Colchicine Intoxication Subsequent to an Autolytic Attempt with Positive Evolution

Background and objectives: Colchicine is a drug that has been used to treat gout for several centuries, it is also indicated in certain autoimmune diseases and has been tested as a chemotherapeutic. Only a few cases of intoxication by this drug have been described, but it is known that it has a narrow therapeutic margin (0.5 to 0.8 mg/kg), almost always resulting in fatal results above these limits.
